An Impacts of Patients¢¥ Visit and Hospitalization on the Enforcement of Health Care Delivery System

Abstract
The number of patients¢¥ visit was significiently increased in general hospital since national health insurance program has been introduced. The goal of the health care delivery system was started to decentralize patients¢¥ hospital visits and hospitalizations based on patients¢¥ disease severity.
This study was designed to fine out changes in numbers of in and out patients to explain characteristics influencing the difference of utilization before and after the start of health care delivery system. Finally the interaction effect between the program and the level of patients¢¥ visiting trend was examined for the above question.
Data was collected from 7 general hospital in Seoul city and 1 general in Pusan city area which were selected randomly.
The following findings from the study were obtained.
1. In-patients¢¥ distribution rates were increased in, such as, neoplasm, 1.21%, disease of the nervous system and sense organ, 0.75%, endocrine, nutritional and metabolic, immunity disorders, 0.63%. But the disease of despiratory system, infectious parasitic, injury and poisoning were decreased.
2. The change was remarkable in disease of the musculoskeletal system and also connective tissue.
3. The increase was shown in surgery, such as, disruption of tissue, 4.87%, eye surgery, 0.88%.
On the other side, the decrease was shown in obstetric surgery, 3.28%, operations on the digestive system, 2.23%, operation on nose, mouth and pharynx, 1.91%.
4. Out patients who visited in pediatrics, internal-medicine, orthopedics were shown 2.34%, 2.00%, and 1.17% each, decreased. But the family medicine, dentistry, ophthalmologic out patients were shown increased 2.02%, 1.93%, and 0.83% each.
5. Even though the rate were decreased in neurosurgery 2.10%, orthopedic surgery, 1.58%, neurology, 1.26%, the total inpatients¢¥ number were increased. This data shows the total number of the inpatients in general hospital has been increased since we have started health care delivery system in 1989.
